Big nose surgery: reducing length, projection and width

In short

A nose that looks big can be reduced, but only in proportion to the rest of the face. The first job is to work out why it looks large: too long, too far forward from the face, too wide, or a mix. Each is treated differently, and the plan also considers the chin, the profile and breathing, so that a smaller nose still looks right.

  • A big nose can be long, projecting, wide or a mix, and each needs a different change.
  • The chin and the forehead affect how large a nose looks, so the whole profile is planned.
  • Thick skin limits how much a reduced nose can slim, and over-reduction can weaken support.
  • Reducing a nose can be done with breathing in mind, and the airway is checked first.

What makes a nose look big?

A nose looks big when one or more of its dimensions is large compared with the face behind it. It may be long from the root to the tip, it may stand far out from the cheeks, or it may be broad across the bridge, the tip or the nostrils. Size is relative: the same nose can look large on a small face and fit well on a broad one. This is why a surgeon talks about proportions and not about a single "right" size.

Bone, cartilage and skin each play a part. A large bony hump, long cartilage in the tip and thick skin all add to what you see from the front and the side. Photos from the front, both sides and below let the surgeon tell which one is the main cause.

Is the problem length, projection or width?

Length, projection and width are different, and mixing them up leads to the wrong operation. The table sets them side by side.

What looks bigHow to tellHow it is usually reduced
LengthThe tip sits low and the nose seems to hang down, especially in the side view. The tip is rotated up slightly, and the septum is shortened with the support rebuilt.
ProjectionThe nose stands far out from the face when seen from the side. The tip and bridge are brought back, with the tip supports resized and re-secured.
Width of the bridgeThe bridge looks broad from the front. The bones are narrowed with controlled cuts, for example with a piezo instrument.
Width of the tip or nostrilsThe tip looks round or the nostrils flare. The tip is narrowed with sutures and grafts, and the nostril base is reduced if agreed.
A humpThere is a bump on the profile that adds length. The hump is lowered and the bridge straightened.

Why do the chin and profile matter for a big nose?

The nose is judged against the forehead, the lips and the chin, so a small chin or a flat forehead can make a normal nose look big. Surgeons check the side view for this balance. If the chin sits back, reducing the nose alone may still leave the face looking unbalanced. In that case the surgeon may suggest looking at the chin as a separate decision, and no promise is made about it.

This is also why a reduced nose should not copy a photo of someone else. The aim is a nose that sits well on your own face and keeps your character. The before and after photos show how the proportions change from one patient to another.

How is a big nose reduced?

The nose is reduced by reshaping its frame from inside, usually through an open approach, so the surgeon can see the bone and cartilage clearly. The operation lasts 2 to 3 hours under general anaesthesia, and the main steps are as follows:

  • The hump and the bridge are lowered, and the bones are brought closer together if the nose is wide.
  • The septum can be shortened and the tip rotated up a little, to take length away.
  • The tip supports are resized and fixed in a new position, to reduce projection without making the tip drop.
  • Sutures and small cartilage grafts narrow and shape the tip.
  • The base of the nostrils is narrowed only if it was agreed at the planning stage.

Details of the usual technique are in the page on primary rhinoplasty.

What are the limits of reduction?

Reduction has a limit set by the skin and by the need to keep the nose strong. Thick skin does not shrink tightly onto a smaller frame, so a reduced nose with thick skin slims less than the same plan would on thin skin, and it takes longer to settle. Taking out too much cartilage can leave the tip weak and the side walls of the nose unsupported, and that can cause both a collapsed look and a blocked airway.

A good plan therefore reduces in steps, rebuilds support where it was taken away and accepts that the result will be a smaller nose and not a very small one. If the nose was reduced before and looks pinched or too small, that is a different problem, treated by a revision operation.

Can reducing a nose affect breathing?

It can, if the reduction is done without thinking about the airway. Shortening or narrowing the nose can weaken the side walls and the nasal valves, so the work has to keep them supported. Dr. Delioğlu is an ENT and head and neck surgeon, so the septum, valves and turbinates are examined first. If the airway needs correcting, it is done in the same operation as a septorhinoplasty, and septal cartilage taken in the process supplies grafts for support.

Can a filler make a big nose look smaller?

A filler cannot make a nose smaller, because it only adds volume. In a few cases a small amount in a low root or a dip above the tip makes the nose look less prominent, and it is temporary. The nose filler page explains what it can and cannot do. For a nose that is long, projecting or wide, surgery is the only real way to reduce it.

What is recovery like after reducing a big nose?

Recovery follows the usual course, with a few differences. The splint stays on for about 7 days, bruising fades over the following weeks, and the whole nose is swollen at first. A reduced nose can look smaller quickly and then seem to fill out again as swelling moves, and the tip is the slowest part, especially with thick skin. Most swelling settles in the first months, and the tip keeps refining for up to a year. The recovery guide gives the general stages.

How much does reducing a big nose cost?

A first rhinoplasty costs $3,000 to $5,500, and a septorhinoplasty that also treats breathing costs $3,500 to $6,000. The range depends on how much has to change and whether grafts are needed. Your exact price comes with a written plan after the photo review, and the cost page compares the operations.

Who may need to wait?

Surgery may not be right for you yet in these situations:

  • You want a very small nose that does not fit your face. The surgeon will explain the proportions that work.
  • Your chin or forehead is the main cause of the imbalance, and you have not yet decided about it.
  • You cannot accept a long wait for the tip, especially with thick skin.
  • You smoke and cannot stop before and after surgery.

What is the next step?

Send photos from the front, both sides and below through the online analysis, and the surgeon tells you which dimension makes your nose look big. You can read about the surgeon on the about page, and the stay is 7 to 10 days in Istanbul.

Who will operate?

Op. Dr. Kemal Caner Delioğlu is an ENT and head and neck surgeon. He graduated from Gazi University Faculty of Medicine, completed his specialist training in otorhinolaryngology in Ankara and worked as a surgeon at Istanbul Başakşehir Çam and Sakura City Hospital before the clinic in Etiler.

The surgeon you talk to online is the one who examines you, operates and follows you up. Consultations are in English, French and Turkish.

Frequently asked questions

It depends on the skin, the structure and the face. The plan shows the change that suits your proportions, not the smallest possible one.

It can if too much is taken. Reducing in steps and keeping support lowers the risk, and proportions are checked before surgery.

Often yes, together with rotating the tip upward slightly. The support is rebuilt so that the tip does not drop.

Sometimes, if the bridge is not wide. A wide bridge usually needs controlled bone cuts to narrow it.

The reduction is possible, but the skin slims less and the swelling lasts longer. The plan allows for it.

Not necessarily. The surgeon looks at the profile and tells you whether the chin matters for your balance.
